Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

About the Role
We are looking for a motivated Health and Wellbeing Worker who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander people. The successful candidate will provide a high-quality service along with contributing to the development of models of holisitic health incorporating continuity of care. As part of Kalwun’s ongoing growth and development, you will be joining a community-focused team at the Coomera clinic.
Position Duties
- Connecting and yarning with clients and families to the service to collect, collate and update their stories
- Provide health information with education to clients and families which promotes autonomy and choice
- Work alongside clients and families in their goal setting and prioritisation of health and social needs ultimately implementing deadly plans
- Responding to clients and families at times of heightened need to prevent and/or avert episodes of crisis
- Make a positive contribution to illness prevention and promotion of health through chronic disease management, health screening, health assessments, recalls and patient education.
Essential Selection Criteria
- Relevant Certificate 111 Qualification (Aboriginal and Torres Strait Islander primary health care, Community Services, Allied Health Assistance or equivalent)
- Demonstrate an understanding of issues affecting Aboriginal and Torres Strait Islander people and their health
- Ability to communicate sensitively and to effectively engage with community built on mutual respect and regard
- Previous experience using patient information systems, (eg: MMEX)
- Demonstrated ability to meet targets and performance outcomes
- Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people
- Demonstrated computer literacy including proficient keyboard and data entry skills
- Ability to work effectively as part of a multi-disciplinary team
- Current Qld C class drivers licence

Additional Information
- This position requires the successful applicant to satisfy a national criminal history check and hold a current drivers’ license.
- A Blue Card for working with children is a mandatory requirement. The ‘No Card No Start’ law applies to this position – a Blue Card must be obtained prior to commencement.
