As a Health Visitor you will devise and implement personalised care plans for each child based on a needs assessment. You will cover the localities of Radstock, Midsomer Norton, Paulton and the surrounding areas.You will join a welcoming and supportive team of Band 6 SCPHN Health Visitors, Band 5 Staff Nurses and Early Years Community Practitioners working to deliver Universal Childrens Services! We work in line with the Healthy Child Programme to improve health, reduce inequalities and safeguard children & young people.We are dedicated to helping you improve your skills and progress in your career with great opportunities for course, apprenticeships as well as a ringfenced £100,000 a year used to implement the ideas of HCRG Care Group employees into practice to improve how we deliver healthcare!This is a full time opportunity!
