About the role

As our case manager, you will support older people living in the ACT to experience their best life. In addition to assessing, building care plans, and supporting clients with their complex needs, you will work with a team of community care workers to deliver the services clients need in line with the consumer-directed/self-directed care model. You will also raise the profile of the service at local expos and other community events.

Duties

  • Work with clients to undertake comprehensive assessments to develop, implement, monitor, and review their client plan using a relationship-centered approach identifying individuals’ and families’ abilities and goals
  • Establish and maintain network relationships with key agencies, other providers, and referral services including ACATS, GPs, Social Workers, Aboriginal and Torres Strait Islander-specific services, and ethnic-specific/multicultural services.
  • Deliver person-centred case management that is based on a collaborative process with the person with a disability, their family, or carers.
  • Ensure simplified access to the disability service system for a person with a disability that matches their goals and/or a family-centred approach with funded service providers and other human service agencies.

Skills and Experience

  • Case management experience in Ageing or minimum cert 4 in Case Management
  • Work in a person-centred, holistic way to achieve outcomes with clients
  • Great people and customer service skills and the ability to communicate with people from all walks of life
  • A natural team player with the ability to balance competing priorities, teamwork, and self-care
  • Outstanding organisational skills with the ability to represent your service at events
  • Drivers License essential
