Job description
The Homelessness Intervention Worker will be part of a dynamic team based at HART 4000 office in Fortitude Valley.
This position aims to connect, refer and communicate with relevant support agencies. It will involve in-house support and active referrals to assist people to link with the services they need.
We are looking for someone with:
- Social Work degree and/or extensive experience in human services
- Demonstrated skills and knowledge of community based intake, assessment, case management & documentation procedures
- Demonstrated excellence in interpersonal, written and organisational skills
- Ability to establish & develop links with communities, communicate & liaise with service providers & work with complex needs clients to provide good case management
- Current valid drivers licence
- Current Police Check
