About the role

The From Homelessness to a Home (H2H) program, in consortium with Haven; Home, Safe, is a Victorian Government initiative to assist people experiencing homelessness to transition from temporary accommodation into safe, sustainable housing in the Barwon region. H2H aims to make a significant, lasting impact on homelessness and rough sleeping by providing clients with access to affordable rental properties, wellbeing supports and tailored mental health, AOD, family violence and physical healthcare services. 

We are seeking a compassionate, resilient Housing & Recovery Coordinator for a 4 month fixed term, part time opportunity to make an impact on homelessness in the Barwon area. You will deliver positive recovery-oriented outreach services to support clients to transition to sustainable housing, develop capacity and daily living skills, and enhance wellbeing to prevent a return to rough sleeping. As this position requires travel across the regional area a fully maintained company car is provided. 

Key responsibilities

  • Deliver high quality outreach services and facilitate positive educational and social experiences.
  • Support clients to source / maintain long-term housing, reduce social isolation and exclusion, and enjoy a meaningful life through a staged approach to recovery.
  • Provide information and referrals to clients to establish connections with families and carers, create community networks and access to relevant services.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals. 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, homelessness issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
