About the opportunity:
Are you passionate about making a difference in the lives of elderly individuals? Doutta Galla West is looking to appoint an In-Home Care Assessment Officer to join our team on a 6-month contract basis. You’ll have the flexibility to work either full-time or part-time (minimum 4 days per week), allowing you to conduct assessments and create care plans for clients benefiting from home care services under the Commonwealth Home Support Program (CHSP) and brokerage agreements.
This role has emerged in response to the growing demand and interest in community care within Melbourne’s west and north-western suburbs. We value your background in community health care, assessment, or aged care, as it will greatly contribute to the success of this position. Additionally, as our services grow, there is the possibility of continued employment opportunities.
About the role:
As an In-Home Care Assessment Officer, you will:
- Manage initial enquiries and provide information on service offerings
- Conduct assessments and complete necessary documentation
- Undertake home risk assessments
- Develop client-centred care plans
- Build and maintain relationships with clients and referrers
- Ensure compliance with service agreements and documentation standards
- Identify opportunities for marketing and promotion
- Participate in quality improvement initiatives and audits
About You:
We’re looking for someone with:
- Excellent communication and interpersonal skills
- Empathy for the needs of elderly individuals
- The ability to collect information in a sensitive manner
- Strong organisational and time management skills
- Excellent skills to collaborate effectively and contribute to the overall success of the team
- High-level competence in the use of Microsoft Suite of applications, including the ability to utilise electronic client management systems and My Aged Care Portal
- Familiarity with the Aged Care Quality Standards Framework
- Minimum Certificate III in Aged Care or relevant experience or qualifications
- Previous experience working in community assessment and/or community care will be highly regarded
- A valid Victorian Driver’s License, and use of a roadworthy vehicle
