About the position

The Intake Officer is responsible for the coordination and assessment of all incoming and outgoing referrals for the Intergrated Team Care (UTC) service providing a streamline and responsive access for patients to be provided with a holistic service approach to meet their health and wellbeing needs. 

The Intake Officer will be the first point of contact for incoming enquiry and will be required to liaise with TAIHS employees, other service/health providers and general practice. 

Duties include:

  • Coordinate the intake of incoming referrals
  • Triage all referrals for risk, safety and patient needs identifying priority of need
  • Facilitate initial assessments for patients 
  • Respond to general enquires regarding service options and referral processes 
  • Ensure referrals are built into the patient Redicase database platform 
  • Contribute to the development of an integrated service model 
  • Complete patient records in accordance with ITC guidelines and certification standards 
  • Provide data to the ITC Senior Care Coordinator as requested 
  • Assist clinical staff to operate in an effective and efficient manner and ensure they have resources required for responsive and respectful service delivery  
  • Develop and document good working relationships / collaborative partnerships with internal and external service providers

To be suitable for this position, your qualifications, experience & licenses for the role will consist of:

  • Certificate IV or Diploma level qualification in a relevant discipline and/or substantial experience in a similar intake office/practice management position or setting within the Primary Care sectors
  • Experience working within a healthcare environment or similar field
  • Highly developed analytical skills and a proven track record in problem solving and effective decision making
  • Ability to work in a highly productive environment with time pressures whilst managing multiple tasks
  • Ability to work both independently and collaboratively as a productive team member
  • Highly developed verbal and written communication skills, and the ability to engage with a diverse range of internal and external stakeholders
  • Advanced computer skills including Microsoft Office, electronic recording systems and data management tools, along with a capacity to learn new clinical software packages if required.
  • Current C Class Drivers Licence (Qld)
  • Satisfactory Police Check – no Serious/Criminal/Court Record
  • COVID-19 Vaccination: It is a condition of employment for this role to be, and remain, vaccinated against COVID-19
  • Current Blue Card (working with children and young people) (or AHPRA registration if required)
