The Care Hub
The Care Hub is a new initiative that brings together a number of organisations to form an integrated team of professionals to support Children and Young People in the areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.

Your role:
You will be joining a collaborative, integrated team of staff helping support those entering care for the first time.
The Key Practitioner within the Care Hub is to undertake assessment, coordinate the care plan including the placement plan and be the one point of contact for the child/ren and young people during their time in the hub. The role will coordinate the multidisciplinary team in coordinating assessment, planning and support children and young people in the hub.
Make your mark:
- Experience managing risk and wellbeing and work within the Best Interest Framework
- An understanding of how trauma can affect the development of young people
- Solid understanding of the Child Protection and Welfare system in Victoria
- Coordination and facilitation of care team meetings and professional meetings with young people, families and other agencies
- Coordinating a multidisciplinary team in assessment, planning and support to children and families
- Development of plans and assessments, report writing, case noting, recording and monitoring compliance
- Monthly statistics
- Provide support to team members and work collaboratively with other teams and agencies ie DFFH, BDAC
Make a move:
- You get a salary starting at around $106,000 plus super
- Salary packaging is available up to $15,900 plus an entertainment card.
- SCHCADS award level 6.
- Negotiable through the increments dependent upon experience.
- Work from the Care Hub in Bendigo.
- Ongoing position.
