The Personal Support and Home Care Service is looking for experienced Personal Support Workers to join our Service Establishment team.
As a Key Worker in the Service Establishment Team (SET) you will meet with new clients interested in our personal care services and will assist with providing information about our service and then help to set up their service by develop/review their routine to create an Individual Support Plan (ISP) and Welcome Onboard service documents. Using the ISP, you will assist with the on-boarding and training of new PSW team members, and work with the client, PSW team and the Client Service Officer to establish the personal care routine and resolving any service or WH&S issues during the establishment of the service.

- Writing and administration skills – developing and writing Individual Support Plans (ISP) Training provided.
- Ability to provide observations and feedback to relevant Nursing and Allied Health staff.
- Person to person training skills – ability to be flexible and adjust your training style and communication to support depending on the client and PSW team
- Problem solving skills
- Flexibility to provide supports to clients in varied locations, sometimes travelling regionally to assist local Client’s and PSW teams.

- Level: Level 4.2 SCHADS
- Tenure: Permanent Part-time
- Hours: Based on new client rosters and operational requirements
- Roster: Rolling Roster – Negotiated 4 weeks in advance to enable work/life balance.
- Location: Based at Woolloongabba Office and travel throughout Brisbane (Applications invited from PSWs in Brisbane central inner south or inner north suburbs however with travel throughout Brisbane.)
