About Us

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria’s largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • Increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • Improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Enhance NWMPHN’s role as a culturally competent organisation by leading the development, implementation and monitoring the use of an access and equity framework.
  • In consultation with relevant NWMPHN staff, lead, develop and implement strategies to improve the capacity of general practice and commissioned service providers to deliver culturally appropriate, safe and inclusive services.
  • Work with the Evaluation and Research Team to use data to understand the impact of commissioning on the delivery of care to vulnerable and diverse population groups.
  • Identify, develop, and implement relevant access and equity training programs and education for internal staff.
  • Lead discussions with business areas to identify emerging population health and wellbeing priorities and provide advice on key issues and changing population needs for vulnerable and diverse population groups.
  • Participate in regular reviews of performance and demonstrate a willingness to close learning and capability gaps.
  • Participate in strategic planning and policy development as per specialist knowledge and skills

We are looking for someone who has:

  • Tertiary qualifications in health, social sciences, or a relevant field.
  • Strong understanding of the strengths and challenges faced by different vulnerable populations about their health and well-being.
  • Minimum of 3 years experience working with vulnerable populations in the health service sector, particularly primary care.
