The Lead, Aged Care Capability Building will provide leadership to North Western Melbourne Primary Health Network (NWMPHN)’s programs to build capability in the aged care sector, including Residential Aged Care Services. The role will work closely to implement health reform initiatives in Residential Aged Care homes, as well as coordinate the NWMPHN Older Adults Collaborative that brings together the programs (both commissioning and capability building) with a focus on improving the health outcomes of older adults in the NWMPHN region.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria’s largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • Increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • Improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Leading, identifying and engaging with community stakeholders including consumers, carers, people with lived experience, service providers and policy makers
  • Leading, supervising and supporting a team responsible for capability building projects in Residential Aged Care Homes (RACH) and help develop coherent annual plans
  • Working with the Integrated Care and Primary Health Care Improvement team to support RACHs to implement health reform initiatives in RACH and to improve integration between RACHs and general practitioners.
  • Coordinating with the NWMPHN Older Adults Collaborative to achieve shared goals and outcomes
  • Implementing commissioning activities using effective project management skills, by effectively monitoring and evaluation strategies that are evidence based, drive the efficiency, effectiveness and quality.
  • Ensuring data is collected effectively, is accurate, accessible and monitored.

We are looking for someone who has:

  • Tertiary qualification in health related or social science
  • Experience working with health professionals and community sector leaders and an understanding of the aged care and primary care sectors, and the current reform initiatives
  • High-level project planning, change-management, people management, evaluation and problem-solving skills, including identification and implementation of workable solutions to problems.
  • Building effective relationships and working collaboratively with internal and external stakeholders to deliver shared outcomes
  • Communicating with influence, including use of written, verbal, presentation and negotiation skills; ability to influence the decisions, actions and perceptions of others and approach negotiations with a convincing and balanced rationale
