The Lead, Integration will be responsible for delivering and coordinating commissioning activities that address service system issues across primary, community, acute and aged care. Reporting to the Manager, Integration, they will work collaboratively with other teams across the organisation and with external partners to realise alignments and opportunities for more integrated and effective primary, community, acute and aged care. This role will primarily focus on the successful delivery of the Care Finders program.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria’s largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Stakeholder engagement including:
    • Leading the Care Finders program on behalf of NWMPHN
    • Completion of contract management activities including: provider performance and relationship management, data analysis, budget reviews, KPI performance management, and reporting.
    • Maintaining strong and collaborative relationships with diverse internal and external stakeholders
    • Facilitating the development of services that are co-designed with stakeholders to address priority needs
    • Facilitate effective engagement, planning, coordination, implementation, and evaluation of commissioning activities that health outcomes for communities across north and west Melbourne
  • Knowledge of project management methodologies to ensure timely achievement of deliverables and performance targets
  • Collecting and analysis of data from service providers and producing high quality plans and reports to meet the requirements of the PHN, the Commonwealth, State, and other funders, as relevant
  • Overseeing other planning and project activities as part of the Integrated Care team and the organisation more broadly, as directed by Manager, Integration

We are looking for someone who has:

  • Tertiary qualification in health, social sciences, community development or related studies and/or relevant ‘on the job’/ professional skills and expertise
  • Broad knowledge and understanding of the issues faced by communities across north and west Melbourne who are experiencing poor physical, psychological, social, and emotional outcomes
  • Strong understanding of the primary allied, and aged care health systems working to support communities across north and west Melbourne and demonstrated understanding of commissioning and ideally knowledge of local services
  • Solution focused approach to problem solving and ability to work on complex programs and identify what is in and out of scope of control
  • Capabilities in effective stakeholder engagement and relationship development with community members, health,community and aged care service providers, policy makers and others
  • Demonstrated experience in project management, including the ability to prioritise actions and negotiate successful outcomes
  • Well-developed communication and interpersonal skills with high levels of emotional intelligence
  • High level organisational and workload management skills, self-motivated and with the ability to work independently and as part of a team to achieve organisational and program specific goals
