About the role
The Mental Health Integrated Complex Care (MHICC) service, funded by the South Eastern Melbourne Primary Health Network, provides specialised supports to clients with severe mental ill health and complex needs who will benefit from improved care coordination over an extended period of time. MHICC supports people that are harder to reach, underserviced or cannot access similar supports under the NDIS and delivers highly flexible outreach services tailored to meet individual needs of clients. Services include clinical nursing services, care coordination and liaison with service providers, family support and improved access to psychiatry and psychological care.
We are seeking a compassionate, experienced Lead Practitioner – Dual Diagnosis for a 4 month fixed term, part time (30.4 hours per week) opportunity to work in the Dandenong area. With work from home options, you will utilise your expertise in mental ill health and alcohol and other drug (AOD) to provide care coordination and outreach services. This role will support clients to build their confidence and capacity, strengthen social and community connections, deliver meaningful referrals to external providers and work closely with clinical partners to achieve recovery goals.
Key responsibilities
- Use expertise in mental ill health and AOD to provide care coordination, high quality outreach services and facilitate positive educational and social experiences.
- Support clients to remain in their community, live independently and enjoy a meaningful life through a staged approach to recovery.
- Offer information and mentoring to clients to build capacity, establish connections with families and carers, create community networks and access to relevant services.
- Provide dual diagnosis practice input to the team in the areas of education, support planning and harm minimisation.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, AOD, Psychology, Social Work, Occupational Therapy or other health-related field.
- Expertise in working with people with mental health issues, AOD dependencies, complex needs and interacting with families and carers.
- Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
- Ability to develop practice and support a team to achieve quality service outcomes.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
