As a Mental Health Practitioner, you will play a key role in supporting individuals with complex mental health needs, delivering specialist assessments, care planning, and risk management within the community. The role requires a strong understanding of safeguarding, social care frameworks, and multi-agency collaboration. You will work closely with service users, their families, and other professionals to develop holistic, person-centered care plans. The position also involves liaising with local authorities, housing, and support services to ensure comprehensive care.Requirements & Qualifications:
To be successful in this role, you will need a current and valid NMC registration (or relevant governing body registration). Community experience is needed. Strong communication skills, the ability to work independently and within a team, and a commitment to high-quality patient care are essential.Working Hours & Contract Duration:
This role offers a flexible working position and requires availability from Monday to Friday with flexibility where possible.
