Role Overview:
- Provide quality mental health nursing care to people experiencing severe and complex mental health concerns in a community-based setting.
- Support clients within a community based setting to provide holistic and coordinated care.
- Development and review of client care and treatment plans to effectively support and monitor wellbeing and treatment goals.
- Provide Brief psychosocial Interventions for clients.
- Work collaboratively with multidisciplinary team members to support clients with their health and wellbeing goals.
- Support the establishment and connection with additional health and mental health professionals, as required.
- Advocate and support client’s health and wellbeing in collaboration with their treating health professionals.
- Provide holistic, wrap-around support and education across all aspects of a client’s health and wellbeing.
Requirements:
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate (essential).
- Current credentialing as a Mental Health Nurse through the Australian College of Mental Health Nurses (desirable)
- Extensive experience working in mental health settings and managing clients with severe and persistent mental health conditions and complex needs.
- Highly developed communication (including written) and interpersonal skills,
- The ability to engage people who are hard to reach and advocate on their behalf
- Establish and maintain effective relationships with staff, clients, carers and stakeholders.
- Ability to use a desktop computer, with familiarity with Microsoft Office applications and capacity to learn how to use a new clinical software package
- Must be eligible to apply/currently hold a Blue Card
- Must hold/be willing to apply for NDIS worker screening clearance
- Current Driver’s License.
