We are seeking an experienced health professional, to join our fabulous team of 23 MND Advisor and Support Coordinators who support people diagnosed with MND across Victoria and Tasmania. This is a 2 year contract role.

In this role you work with those impacted by motor neurone disease to:

  • provide advice and support to help provide an understanding of the disease and its impact
  • Undertake holistic and timely assessment
  • Identify, refer and negotiate services to address current and future need
  • provide support coordination for NDIS participants

The successful applicants will have:

  • experience in working with people living with a progressive degenerative neurological disease
  • excellent knowledge of support and care planning, assessment and referral in the disability and/or aged care sectors
  • a “can do approach” with a demonstrated client focused approach to all work activities
  • a relevant tertiary qualification in a health/welfare related discipline, e.g. an occupational therapist, social worker, physiotherapist, speech therapist, nurse or psychologist

This role offers diversity and variety working across NDIS for those clients under 65, and My Aged Care for those over 65.

We require the successful applicant to have experience working with people living with a progressive degenerative neurological disease, however, are able to offer extensive training about motor neurone disease itself.

This is a full-time role on a 2 year contract.

Excellent salary range $107,939 to $112,810, a work car (with ability to use for reasonable personal use) and salary packaging available.
