About the Role:
As a Support Coordinator, you will support people with a disability to implement their NDIS plan and link them with other service providers to ensure their support needs are being met. you will support people with a disability to implement their NDIS plan and link them with other service providers to ensure their support needs are being met. You must have an excellent work ethic, strong time management skills and have a high degree of creativity. You will manage your own caseload of NDIS participants, while using your person-centred ability to establish rapport and promote professional relationships.

Key Responsibilities:
- Assist participants to understand and navigate the NDIS. Working closely to ensure funding is utilised in line with NDIA guidelines and personal goals are achieved.
- Link your customers to services that build on their strengths and support them to work towards their goals
- Help them exercise choice and control
- Connect to community and mainstream services in order to provide a well-rounded service that ensures progress and outcomes for customers.
- Maintaining regular and accurate customer information, documentation and reporting that meet legislative requirements.
- Assessing and managing risk in line with organisational policies and procedures to ensure customer safety.
- Support participants and their service providers for plan reviews
- Negotiate services and prices as part of any quotable supports
- Arrange any assessments required to determine the nature of required funding (e.g. Home modifications required, Assistive technology).
Essential Criteria:
- A minimum of one year experience in support coordination or other relevant roles within the NDIS sector.
- Qualifications in Disability, Health, Community Services, Nursing, Social Science, Mental Health or equivalent
- NDIS Screening Check (state based)
- Current driver’s licence.
- Valid work rights in Australia.
- Current National Police Check (within 12 months validity), (or be prepared to obtain)
- Current First Aid & CPR certification (preferred)
- NDIS (National Disability Insurance Scheme) Worker Orientation Module Certificate.
- Comprehensive knowledge of the NDIS, community and mainstream services.
- Good time management ability and ability to meet target billable hours.
- High level of organisational and time management skills.
- Ability to work effectively as part of a diverse team and independently.
- Highly developed oral and written communication and interpersonal and negotiation skills with the ability to communicate effectively with people from diverse backgrounds.
- Strong computer and administration skills.
