We are seeking qualified and passionate Support Coordinator. You will play a pivotal role in supporting people with disabilities to improve their quality of life by assisting them to understand and implement their NDIS package. This includes establishing positive relationships with a broad range of service users and stakeholders, developing and implementing support plans, and coordinating with a wide range of services.
Be part of a supportive team whose focus is to build supportive, long-term, respectful relationships with people experiencing psychosocial or other disabilities. Our Offer

- Salary of $82,326.30 p.a. plus 11% Superannuation

Experience & Qualifications
- Minimum Certificate IV in Disability, Mental Health, Community Services, or similar
- Effective oral and written communication skills
- Experience providing outreach, case management, or similar
- Demonstrated ability to establish rapport and build and maintain sound relationships with individuals, carers and families
- Demonstrated ability to be able to manage multiple competing priorities, have excellent time management skills and in a fast-paced environment.
