In this role, you will work collaboratively with multidisciplinary teams to provide tailored support and interventions to individuals experiencing mental health challenges. Your work will be pivotal in helping individuals achieve stability, independence, and improved well-being.Key Responsibilities
Undertake assessments of individuals experiencing mental health difficulties under the Care Act 2014.
Develop, review, and implement care plans that promote recovery, independence, and social inclusion.
Work collaboratively with health professionals, families, and carers to ensure a holistic approach.
Act as a lead professional, coordinating support and intervention where necessary.
Conduct safeguarding investigations and risk assessments to protect vulnerable individuals.
Maintain accurate and up-to-date case records.
Ensure compliance with statutory regulations and best practices in mental health care.What We’re Looking ForEssential:Degree in Social Work or equivalent qualification.
HCPC/Social Work England registration.
Proven experience working in mental health settings.
Strong knowledge of relevant legislation, including the Care Act 2014 and Mental Health Act 1983 (amended 2007).
Excellent assessment, communication, and interpersonal skills.Desirable:AMHP (Approved Mental Health Professional) status or willingness to train.
Familiarity with Newcastle’s local health and social care systems.
