The Department of OBGYN has an exciting opportunity for a full-time Nurse Navigator to work in Miami, Fl.  The Nurse Navigator is the initial point of contact for a patient entering the health system. The Nurse Navigator assures timely scheduling of the first appointment, coordination of care after completion of the first appointment, and will support the patient throughout the care continuum. The role also serves as a liaison between patients, families, caregivers, and the multidisciplinary care team.  Furthermore, the Nurse Navigator demonstrates commitment to quality patient care, implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices.         

  • Triages new patient appointments to the appropriate provider(s) and assures timely scheduling of initial appointments.

  • Reviews outside medical records for appropriate scheduling.

  • Assess barriers to care and refers to support services, local, and national organizations when needed.

  • Educates on the treatment plan for patients based on diagnosis.

  • Supports patients throughout the care continuum.

  • Counsels individuals and patients on positive health practices.

  • Collaborates with a multidisciplinary team of experts to outline best treatment for patients.

  • Performs holistic evaluation of specialty population, making use of enhanced proven techniques and procedures to achieve better results.

  • Implements the improvement of patient care, and healthcare policies and resources.

  • Mentors other healthcare professionals by functioning as a preceptor or coordinating preceptors for visiting professionals, students, new graduates, and orienteers.

  • Maintains professional knowledge by affiliating with professional and technical organizations, and participating in applicable continuing education programs, conferences, seminars, and workshops.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

MINIMUM QUALIFICATIONS                                                                                                         

  • Bachelor’s degree in relevant field

  • Valid State of Florida Registered Nurse (RN) license

  • Minimum 2 years of relevant work experience

  • Ability to maintain effective interpersonal relationships.

  • Ability to communicate effectively in both oral and written form.

  • Skill in collecting, organizing and analyzing data.

  • Proficiency in computer software (i.e., Microsoft Office).

  • Bilingual strongly preferred – fluency in English and Spanish

  • Expert clinical critical thinking skills

  • Strong conflict management and resolution skills.

  • Be sympathetic and provide emotional support to the patient.

  • Able to work in tough stressful condition with a positive attitude.

  • Outstanding interpersonal skills, ability to handle and resolve sensitive patient health information.

  • Requires the ability to work independently and to accept responsibility for complex and sensitive decision-making as it relates to the overall flow of the post care practice.

DEPARTMENT PECIFIC FUNCTIONS                       

  • Answer and responds to calls in a timely fashion to facilitate attending to patient needs.

  • Retrieve clinical related messages and return calls within 24 hours.

  • Manage and resolve UChart messages.

  • Complete lab order requests.

  • Complete prescription refills.

  • Process authorizations for medications.

  • Interprets and responds to laboratory test results of patients as required.

  • Uses clinical protocols to refer patients to the appropriate specialist for subsequent diagnosis or treatment.

  • Practices from a Relationship-based Care Delivery System, makes the patient and family feel known and cared for, and provides emotional support and counseling.

  • Responsible for developing and enhancing processes with the aim to improve the clinical experience for referred patients or physicians.

  • Assists patients and their families in obtaining referrals to a specific specialist.

  • Serves as the liaison between patient and provider.

  • Takes care of all patient requests.

  • Other duties as assigned. (Time spent on the Nice system /phone is 90% of the day.)
