Position Description:

Position Purpose

Advanced Practice Nurse

Provides primary or specialty health care services within defined scope of practice in collaboration with a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. 

The APP/NP work in partnership with all Pediatric Hospital Medicine physicians, the Medical Director of the PHM-FV program and the Assistant SPU Chief, who serve as the primary collaborating/supervising physicians, the Medical Director of PHM-FV, Assistant SPU Chief-FV, Director of PHM-MKE and the Division Administrator II – PHM-MKE. 


Core Responsibilities

  • Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.
  • Orders and interprets laboratory and diagnostic tests.
  • Develops pharmaceutical and non-pharmaceutical treatment plans.
  • Monitors effectiveness of plan of care.
  • Educates patients on disease and treatment plans.
  • Performs procedures as trained and appropriately credentialed.
  • Documents within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborates and/or consults with physicians and other health care providers as appropriate.
  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.
  • Department Specific Responsibilities

  • Patient Care:

    • Collaborate with CW-FV PHM team and interdisciplinary health care teams in caring for newborns born and cared for at Thedacare-Neenah hospital as well as:
    • Children presenting for infusions/short stay or other care at CW-FV hospital
    • Order and interpret diagnositic tests, procedures, laboratory studies and professional consultations. Appropriately communicate findings with the medical team as well as with patients and families.
    • Develop and implements treatment plans in collaboration with the PHM attending, monitor effectiveness of plan of care throughout the patients hospital stay, coordinate the care and discharge of patients.
    • Document within the EMR accurately and timely the Admission, Progress and discharge notes as well as orders and discharge instruction as well as communicate with the PCP as needed.
    • Support the infusion clinic with treatment plan implementation, order entry and participate in meetings regarding this clinic.
    • Additional clinical care functions as this role develops within the APP/NP licensing limitations and clinical skills.

    2. Teaching/Education:

    • Provided education and counselling to patiens and families on well newborn care, applicable disease and treatment plans.
    • Provide instruction (formal or informal) to nurses, nursing students, medical students, residents, fellows and other health care professional trainees.
    • Attend required training, educational sessions, PHM specific meeting and any committee meetings you participate in.

    3. 

    Administration/Service:

    • Participate to the extent that you are able in administration and /or service functions of the Department.

    4. Evaluation

    :

    • Will be conducted monthly for the first 3 months, every 3 months for the next 6 months and every 6-12 months thereafter.

    Work Schedule: 

    Full Time (1.0 FTE): 40 hours of scheduled patient related work per week. Up to 5 hours per week of work related to citizenship, team representation, and organizational participation. Work hours are 7:00-16:00 5-days per week which may include some weekend days depending on scheduling needs. Schedule and hours may be modified to accommodate the program’s needs. Non-clinical hours are included and will fluctuate depending on needs and tasks required. Part-time options will be considered.


    Preferred Schedule:


    Full-time role with expectations for coverage during core business hours of 7:00-16:00 M-F, including some weekend days with flexibility


    Position Requirements:


    Minimum Education: 

    Advanced Practice Nurse

    Master’s Degree or higher in Nursing

    Prior Experience: 

    • No experienced required
    • Prefer one or more years of experience in pediatric acute care and /or newborn care.
    • Prefer experience using EPIC or other electronic medical record

    Certifications / Licenses

    (Must be current and maintained)

    Advanced Practice Nurse

    • Wisconsin RN license
    • Wisconsin Advance Practice Nurse Prescriber License
    • Certification as Advance Practice Nurse

    Department Specific Certifications / Licenses

    • Pediatric certification required.
    • DEA
    • NRP certification

    BLS and PALS certification
