Jewish Care Victoria, is a not-for-profit organisation and is one of the largest providers of aged and community services for the Jewish Community in Victoria and has been servicing the community for 175 years. Our staff come from diverse cultural and religious backgrounds. We offer an Ethos Day – an enriching learning experience that delves into the intricacies of Jewish history and culture. About you
Play a key role in enhancing the independence of our aged care service users, including those with Home Care Packages and CHSP funding. As part of our dedicated Active Living Services team at Jewish Care Victoria, you will make a positive impact by supporting individuals and groups using an evidence-based approach to deliver effective interventions.
The program offers essential assistance, connects clients with community resources and ensures access to ongoing services to address a wide range of health needs.
Primarily based in our state-of-the-art Residential and Health & Wellbeing facilities in Windsor and Caulfield, you’ll have the opportunity to work across a variety of community sites and in clients’ homes, with a strong emphasis on empowering individuals to live independently and actively.
What you will do
- Initial assessment of clients and development of individualised care plans
- Perform client reviews using appropriate outcome measures
- Promote independence and functional abilities for both residential and community clients, with a strong focus on health promotion
- Communicate client outcomes through attendance at relevant clinical/team meetings and documentation in the client data base
- Supervision of occupational therapy, medical and other relevant allied health students
- Promotion and understanding of cultural diversity in care services
You will have:
- Degree in Occupational Therapy
- Registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- Demonstrated knowledge of a Wellness Reablement approach to service delivery in community aged care
- Experience in the development, facilitation and evaluation of group programs
- Proven experience and strong clinical skills gained through working in community aged care
