The opportunity
We are currently looking for an experienced community Occupational Therapist to support our clients throughout the Moorabool Shire.
In this role, you will be responsible for the delivery and coordination of professional Occupational Therapy services across a number of programs (Reablement, Commonwealth Home Support Programs and Home Care Packages) to achieve improved occupational, social and health outcomes, optimum independence and improved quality of life for our clients.
Working in a community aged care setting, you will work independently completing home-based assessments and provide appropriate recommendations, intervention and evaluation. Experience in care management and transdisciplinary practice would be an advantage.
This role will be based out of our Maddingley, Victoria 3340 office however due to the nature of the service you will spend time traveling to deliver care to our clients in their own homes. A pool car, the technology required to do your role, and orientation to and support in the role will be provided. Regular PD inhouse and peer supervision is available.
The hours of work in this role are flexible. We are able to consider candidates with availability Monday to Friday, with a minimum of 1 day a week. This position may suit working parents with family responsibilities.
Key Responsibilities
- Grade 2 Occupational Therapist role
- Community based role with a strong focus on enhancing the independence of our clients
- Plan and carry out required services to clients following guidelines for assessment and treatment required
- Maintain a close liaison with other community resources to ensure the ongoing care
- Liaise with families/care providers by providing ongoing support, counselling and education
- The ability to work independently
- Plan and carry out relevant training to staff
- Prepare reports as required
Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
About you
To be considered for this role, you will meet the following criteria:
- Previous experience working as an OT in a community setting
- Tertiary qualifications in Occupational Therapy
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Ability to identify and provide appropriate advice on complex care needs by exercising initiative and innovation in the application of professional practices under limited direction.
- Previous experience in minor home modifications and equipment prescription
- Ability to use a reablement / restorative model of care to enhance the independence of our clients
- Excellent interpersonal and communication skills (written and verbal)
- High level computer skills
- Ability to work autonomously as well as work well within a multi-disciplinary team
- Highly organised, strong attention to detail and ability to meet deadlines
- Valid Australian drivers licence
