This position functions to assist the medication safety pharmacist in providing a safe environment for patients, staff members and visitors. Interns will review event reports to determine contributing factors to errors, severity of the harm to the person affected and probable solution(s) to prevent errors from occurring in future. Under the guidance of the medication safety pharmacist, interns will help with data collection, data analysis, data presentation and discussion of various patient safety projects. This position is also responsible for supporting initiatives of the medication safety pharmacist and other preceptors including regulatory compliance, policy maintenance, quality assurance and auditing

85% Medication Safety Core Activities

Under the direction of the medication safety pharmacist, in coordination with pharmacy and non-pharmacy staff, and according to applicable state and federal law, interns will:

  • Learn the pharmacy work flow and patient care processes to perform event reviews and projects.
  • Utilize various resources (policies, algorithms, external links of databases, service coverages, schedules, etc.) on pharmacy intranet, shared drive and OneSource to aid in events review and projects.
  • Review three categories of events – medication related, adverse drug reaction and extravasation.
  • Generate reports using PSRS system
  • Use data analysis tools such as Microsoft Excel’s pivot table to organize and interpret collected data in an efficient and meaningful manner.
  • Create slides and deliver presentations to the Pharmacy and Therapeutics Committees & other quality and safety-focused teams.
  • Interact with physicians, nurses, and other allied health professionals in a courteous and professional manner.
  • Console staff member involved in error according to the Just Culture principle when appropriate.
  • Demonstrate the ability to work as a member of a team by exhibiting effective interpersonal skills, remaining flexible and open-minded.
  • Demonstrate the ability to work independently when appropriate

                                                                                                         

10% Teaching/Training/Educational Activities

  • Participate in the orientation and training of volunteers, pharmacy technicians, pharmacists, undergraduate and graduate students, and other hospital staff.
  • Attend department sponsored continuing education programs Satisfy life-safety training and other training and education requirements
  • Meet annual competency requirements for department
  • Be committed to personal growth and development
  • Complete pharmacy technician general training and educational modules/programs
  • Work with area managers and pharmacists on projects and department initiatives as the needs arise

5% Productivity Management

  • Maintain organized workspace.
  • Proactively seek solutions to patient, staff and visitor’s problems encountered both inside and outside of the department
