Scope of Position In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel.  Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests.  Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department) , drug-disease state and drug-laboratory interactions.  Participates in appropriate hospital and/or departmental committees relevant to practice area.

Position Summary This position functions to coordinate all aspects of patients’ pharmaceutical needs in the patient care areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients’ pharmaceutical needs with physicians and nurses.  Patient population includes adult, geriatric and occasionally neonatal patients.  Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.  Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.  This position is also responsible for supervising technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).

Duties and Responsibilities Dispenses medication pursuant to a physician’s order according to the laws and regulations as defined by the Ohio State Board of Pharmacy and The Ohio State University Wexner Medical Center’s drug distribution policies.  Enters or reviews entry of medication orders into the pharmacy computer system.  Oversees appropriate packaging for drugs including oral, parenteral, ophthalmic, intra-arterial, intra-ocular, topical and investigational medications.  Prepares and supervises the preparation and labeling of medications for dispensing items identified above.  Compounds or supervises compounding of special preparations and parenteral medications.  Maintains patient mediation profiles, which include drug allergies and disease states.  Monitors the accuracy of drug dispensing.  Fulfills all applicable MUE requirements. Complies with OBRA 90 requirements and accreditation standards applicable to pharmacy practice and employment.

Approves new drug orders in collaboration with other pharmacists, technicians, nurses,

             and physicians

Screens for drug interactions (including drug-drug, drug-herb/dietary supplement, drug-food interactions in collaboration with Registered Dieticians)

Fulfills all applicable MUE requirements.

Prevents, detects, manages, and documents adverse drug reactions to minimize adverse events in patients

Maintains accuracy and timeliness in drug dispensing to ensure a high level of customer service

Responds to drug information requests from all internal and external customers.

Performs patient education in collaboration with nurses, physicians, and other health care professionals

Completes pharmacokinetic and therapeutic consults and recommends alternative therapy choices when appropriate.

Evaluates physician’s medication orders for appropriateness

Maintains competency by attending seminars, CE programs, and through outside readings

Participates on the Cardio-Pulmonary Resuscitation Team as required and subsequent to appropriate training

Participates on hospital and/or department committees

Presents ideas to managers for process and quality improvement

Interacts with physicians, nurses, and other allied health professionals in a courteous and professional         manner

Demonstrates ability to work as a member of the team by exhibiting good interpersonal skills and                remaining flexible and open minded

10%     Teaching Educational Activities

Serves as a preceptor and/or instructor to graduate and undergraduate pharmacy students

Maintains competency of staff by providing continuing education programs to hospital and/or pharmacy department staff

Minimum Qualifications

For Hire:         Graduation from an ACPE accredited College of Pharmacy and licensed to practice pharmacy in the State of Ohio.    Strong preference is given to candidates with Pharm.D degree

On going:         Performs supervisory support functions outlined above.  Continues to meet competency and licensure expectations of a staff pharmacist.

Job Relationships

Supervisory Responsibility: Supervises pharmacy technicians, pharmacy interns and new pharmacists training in the area.  Provides information relative to staff performance for use in performance evaluations.  Documents and discusses inadequate performance with the above individuals when appropriate (i.e., tardiness, improper techniques, etc.)

Contacts: Physicians, nurses, pharmacists, pharmacy technicians, pharmacy residents and undergraduate students.  Weekly contact with administrative personnel in the Pharmacy Department

*          Able to perform the following tasks  (i.e. filling prescriptions, preparing IV admixtures, monetary transactions)

*          Frequently filling prescriptions, placing stock

*          Constantly answering telephones, computer order-entry, billing functions

*          Frequently obtaining medications from stock.

*          Constantly obtaining medications from various heights (floor to 8′), obtaining supplies, preparing IV admixtures.

*          Constantly handling  (i.e. separating IV admixtures, grasping syringes and other supplies, opening medication bottles, answering telephones)

*          Constantly required to work with unit dose pharmaceuticals, dial telephones, manipulate syringes and other pharmacy supplies, package pharmaceuticals, use a typewriter, and a computer keyboard)

*          Able to produce clear, intelligible speech 100% of the time (i.e. communication with hospital and pharmacy staff about medication orders, and also patients)

*          Understands verbal communication 100% of the time, (i.e. communication with hospital and pharmacy staff about medication orders, and also patients)

Work Environment  

The work involves risks or discomforts which require special safety precautions, additional safety education and/or health risk monitoring, e.g. working around moving parts, carts, machines; working with sharps (i.e. needles, syringes, etc.), medications (i.e. chemotherapy, antibiotics, ionizing radiation, etc), chemicals (i.e. laboratory and/or cleaning agents).  Employee may be required to use protective clothing or gear such as masks or gloves.
