The Program Officer, Quality Improvement will be responsible for the implementation of the Primary Care Pathways to Safety Project. This project provides tailored support to primary care providers, to recognise, respond and refer people at risk of, or experiencing, family violence. The project will also improve collaboration and build greater cohesion and coordination across a range of local health, social care and family violence services.

The Program Officer, Quality Improvement will also have a key role in working in a planned, proactive and accountable way to enable general practice to deliver quality care. This will be achieved through supporting general practice to implement quality improvements and relevant health reforms, capturing innovation and learnings for translation into other programs and activities.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria’s largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Maintaining high performing working relationships with internal and external stakeholders, to deliver outcomes within services and programs
  • Contract management for service providers, education and training providers or general practices
  • Coaching practice teams to use and embed quality improvement tools and methods within general practice
  • Collaborating and integrating activities across the organisation to achieve shared goals and enhance general practice engagement and relationships
  • Maintaining up to date practice data and contact information in NWMPHN systems

We are looking for someone who has:

  • Qualifications or relevant experience in public or not for profit sector, primary health care or health care
  • Project management experience – experience in scoping, planning, monitoring, and reporting on projects
  • Demonstrated capability for quality written communication, including the preparation of external communications and reports to a professional standard
  • Demonstrated relationship management skills including the ability to develop and maintain strong collaborative partnerships and to work with all levels within the workplace
  • Demonstrated understanding of using and interpreting data to guide improvement
  • Ability and competence in the use of technology and a variety of software systems, including competency with MS Office (Excel, Word, PowerPoint)
