Job Summary:

The RN Nurse Educator is responsible for exemplifying the University’s Mission, Vision, and Values providing outstanding customer service. This position is an integral part of the interprofessional teams serving as a coordinator and trainer, working closely with the Program Director and the Lifestyle Change Program staff to assist with coordination, implementation, and training of internal and external clinical use of EPIC CareLink.

This position will be responsible for coordinating prevention and self-management of chronic disease (cardiovascular disease and diabetes) opportunities for internal and external clinics within six (6) specified census tracks assigned by Texas Department of State Health Services. The RN Nurse Educator will engage in collaborative efforts such as awareness and training to healthcare providers to increase acquisition of patient/participant referrals through EPIC CareLink bi-directional referral system. This position delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care.

The RN Nurse Educator provides collaborative, comprehensive, and education conducive to behavior change and improved quality of life across the lifespan to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. This position requires the ability to function effectively as a team member and make moderately complex decisions with limited supervision.

 

Major Responsibilities / Duties / Critical Tasks:

·         Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.

·         Advocates for and communicates about improved quality of care and outcomes for those living with and/or at risk for and affected by diabetes and cardiometabolic conditions.

·         Coordinate, develop, implement, and train internal and external clinical staff on utilization of EPIC CareLink in efforts to promote awareness and increase acquisition of bi-directional referrals through EPIC CareLink.

·         Lead training and consulting strategies for EPIC CareLink to internal and external clinics and providers within six (6) designated census tracks.

·         Lead strategies to implement clinic assessment and development of diabetes care and education specialist (ADCES) program and/or recognition by the American Diabetes association (ADA).

·         Assist with quarterly reporting.

·         Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.

·         Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan.

·         Applies business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).

·         Engages in lifelong learning and serves as a role model of professionalism.

·         Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.

·         Collaborates with community partners to establish and maintain on-going support options.

·         Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

·         Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.

·         Other duties as assigned.

Qualifications

 

Required Education / Experience:

·         Bachelor’s degree in Nursing.

·         Diabetes self-management education and support experience preferred.

 

Required license/certification:

·         Valid Registered Nursing License.
