About the role

Our Fostering Young Lives program supports children and their foster carers throughout Sydney. Our services offer ongoing and coordinated support to best meet the needs of children and their carers, so they feel supported in the work they do. As a Senior Case Manager, you will support Case Managers in performing their role within this program, assisting them with preparations for visits and ensuring high quality service delivery. You will build team capacity and work with more complex client needs and work intensively with children, young people, and careers.

Duties

As a Senior Case Manager in the Fostering Young Lives programme, you will:

  • Work with a number of complex cases, supporting children, young people and their carers, and undertake comprehensive assessments to develop, implement, monitor and review case plans
  • Support other Case Managers with their complex cases, including attending visits, meetings, and to resolve more challenging situations
  • Develop contact schedules and conduct regular contact visits as a key worker
  • Coordinate with other service providers, including assisting clients to engage with wrap around services directly, as appropriate
  • Attend review meetings, case conferences, worker meeting and consultation meetings and ensure appropriate documentation is kept
  • Coach, advise and provide on the job support to Case Managers, Counsellors, and others to improve client outcomes
  • Support the leadership team with learning and onboarding needs and develop a range of solutions to build capacity within the team outcomes

Skills and Experience

We want you to be successful in your new role! This means, as an experienced Social Worker, Psychologist, Case Worker or Case Manager you will have

  • Degree qualified in social work, psychology or similar
  • Extensive experience in case management, particularly in child protection
  • Strong grasp on child protection issues, including substance abuse, domestic violence and trauma’s impact on a child’s development
  • Familiarity with evidence-based practices and intervention models, such as developmental and attachment theories
  • Computer literate with excellent verbal and written communication skills
  • Skilled in building relationships with stakeholders
