We’ve launched Living Better Lives in Lancashire – our new strength-based vision for social care.We’re driving forward innovative practice that will make a difference to our communities and we need new senior social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.The role…Our exciting, yet fast-paced Mental Health Social Care Hospital Discharge Team requires an accessible and proactive leader to guide our Social Workers through challenging cases and daily tasks. This role involves working with adults with a range of mental health difficulties who require statutory interventions. You will undertake complex assessments, commission care, undertake duty tasks, and work as part of the wider multi-disciplinary team with a focus on collaboration and co-production.The post requires integrated and collaborative working as part of a team, supporting Lancashire residents being discharged safely and timely from the hospital wards in Blackburn, Blackpool, Chorley, Lancaster, Ormskirk, and Preston. You will also be required to assist the Team Manager with complaint handling, recruitment, supervision, and delivering Best Practice sessions to your team to promote continued development.You will need plenty of energy and motivation, with a “get up and go” attitude, to thrive in this role. Our team values a certain flair and finesse when dealing with challenging clients; every case is unique, so a person-centred approach will be your key to success. Although you will be pivotal in providing support to your fellow Social Workers, you will also receive unwavering assistance from your line management team and the wider organisation, enabling you to perform at your best.We offer a range of development opportunities and support progression, including the opportunity to train as a Practice Educator.
