About the role
We are seeking a highly experienced and culturally competent Senior Support Coordinator to join our social care team in Mardi, New South Wales. The successful candidate will have a strong background in disability services, case management, and working within the NDIS framework.
They will be responsible for developing, implementing, and monitoring NDIS plans to meet the individual goals of participants, with a particular focus on supporting Aboriginal and Torres Strait Islander communities.
The role requires excellent communication skills, organisational abilities, teamwork capabilities, and a commitment to continuous improvement and learning.
Skills required:
- Demonstrated experience in disability services or related field
- Understanding of NDIS processes and plan management
- Cultural competency in working with Aboriginal communities
- Excellent verbal and written communication skills
- Strong organisational and time management abilities
- Ability to work effectively in a team or independently
Responsibilities:
- Develop individual NDIS plans for participants based on their goals and needs
- Implement support coordination services to ensure plan goals are met
- Monitor progress of participants’ plans regularly
- Collaborate with healthcare professionals and community services for comprehensive support
- Advocate for participants’ needs within the NDIS framework
