Your new role

New vacancies exist for highly motivated Social Worker with appropriate knowledge and skills to join our friendly and supportive team. As a Social Worker, you will be working in Sacred Heart Local’s (SHL) In-Home support team.

Your role will be within the Commonwealth Home Support Program (CHSP), delivering supports specifically tailored for people who are aged, homeless, at risk of homelessness and /or socially disadvantaged. SHL deliver accessible, responsive, evidence informed and innovative services with this client group in the community to safely maintain their familiar environment and independence for as long as possible.

This Social Work position will be co-located at Vincent Care’s Northern Community Hub (Glenroy). The role will provide outreach support to clients across the Northern Metro Planning Region.

Your day-to-day

A typical day will involve working in the office and visiting clients in their own home. 27 hours a week, ideally worked across four days, however, flexible working arrangements could be made with the right candidate.

Social work focus will have a networking, advocacy and linkages focus, assisting older people experiencing homelessness and disadvantage to engage in clinical and social supports via comprehensive assessment, emotional support, service coordination, and referral.

You will undertake assessments, care planning and coordination activities, using trauma informed care and social inclusion principles. You will ensure the best outcomes for your clients by working closely with colleagues, the support team and local services and agencies.

We’re looking for someone

The successful candidate will:

  • Have a tertiary qualification in Social Work from an accredited Australian Association of Social Workers provider
  • Have extensive Social Work experience in the Community Aged Care or Homelessness sectors
  • Experienced in client engagement in an outreach capacity
  • Outstanding written and verbal communication skills, as well as strong interpersonal skills
  • Be compassionate and motivated to help people maintain their independence
  • Have the ability to participate collaboratively within teams of peers, staff and other stakeholders, as well as initiative and effective personal judgement when working alone
  • Have a driver’s license and access to your own car
  • Have a current first aid certificate including CPR
  • Have experience with multiple and complex care needs (desirable)
