About the Position
We are looking to welcome a Specialist Family Support Practitioner to deliver an outreach model of complex family case-management programs to families, caregivers, kids, and young people.
- Full time or part time (0.8FTE) opportunity.
- This role is government funded until 30 June 2027, with possibility for extension.
- Level 5 under the SCHCADS award. Salary range is $93,860 – $98,112 (pro-rata for part-time) plus super.
Key Responsibilities:
- Maintain a caseload of families and provide evidence-informed case management interventions for clients with varied complex needs specifically, those whose clinical presentations incorporate mental health and/or disability.
- Engage families with complex needs, obtaining consent and developing a case plan in conjunction with the family which addresses unmet needs and child protection concerns.
- Work collaboratively with other professionals and agencies to ensure families goals are achieved.
About You
- To perform this role, it is essential that the person who holds the position be an Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 that applicants are Aboriginal or Torres Strait Islanders.
- Willingness of a respected Aboriginal and/or Torres Strait Islander cultural referee to vouch for your appropriateness for this role.
- A tertiary degree in a relevant discipline (allied health, social work, human services).
- Professional registration (if required) and membership of a relevant professional association (desirable).
- At least five (5) years’ experience providing complex and intensive case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework.
- Experience working with people with mental health, disability, and complex needs, and interacting with families and carers.
Other Requirements
- Eligibility for a Working with Children Check or professional exemption.
- Legally able work in Australia (proof of eligibility will be requested).
- Fully vaccinated against COVID-19 or have an approved medical exemption.
