We support people with complex disability and mental health issues and people who are neurodivergent. Our team of experienced psychologists, therapists, PBS practitioners and specialist and support coordinators provide support, therapy and learning so that people can reach their full potential. We also provide training and capacity building to organisations in the community sector.

You will be working within a very supportive team and assisting clients by:
- Forming ongoing relationships with clients by listening, building rapport and trust.
- Ensuring clients are aware of their rights and can self-advocate or access advocacy for what they want and need, this includes, offering choices and empowering participants to make decisions around their supports.
- Working to ensure cultural safety.
- Supporting and understanding psycho-social disability whilst navigating the mental health system within the NDIS framework.
- Working in a trauma informed and person-centred approach.
- Adapting to different situations and crisis responses such as; attending VCAT hearings, supporting clients to access housing, facilitating hospital and incarceration discharges and accessing mental health services and other mainstream supports.
- Communicating with different service providers (e.g., Allied health professionals, VACCA, Office of the Public Advocate Guardians, Behaviour Support Practitioners, Support Workers, Community Programs, Justice Workers).
- Report writing and gathering appropriate evidence to advocate for adequate funding that meets client needs.
- Facilitating or attending Care Team meetings or meetings with the NDIS to discuss the ongoing client needs, concerns or funding requirements.
About You:
- Bachelor’s Degree or post graduate qualifications in psychology, forensic, criminology, occupational therapy, social work, speech pathology or a similar field and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice)
- Current membership or eligibility to join relevant regulatory body to your degree (e.g., AASW, AHPRA)
- Relevant experience providing Support Coordination or Case Management to complex clients such as homelessness, justice, child protection, disability and mental health.
- Demonstrated ability to develop positive relationships with a range of clients, their families, and their service providers.
- NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants.
- Experience working with vulnerable and disadvantaged groups in the community.
- Awareness of services such as homelessness services, mental health supports, mainstream housing options, justice system.
- Excellent communication skills including; ability to build rapport, case noting, report writing and facilitation of care team meetings.
- Cultural awareness and ability to apply and integrate this into your everyday work.
