EACH is proud to be the Lead Agency of the Mental Health, Alcohol and Other Drugs, and Suicide Prevention (MHAODSP) program, a new initiative delivering comprehensive, integrated, and person-centred care across the CCQ region (Central Queensland, Wide Bay, Sunshine Coast).

Collaborating with 9 partnering organisations the aim of this service is to address service gaps by enhancing accessibility and integrating primary mental health care, AOD, psychosocial support, and early intervention and prevention through a multidisciplinary, person centred model of care. Job Description 

The Stepped Care Lead is a pivotal role in the delivery of a new collaborative community mental health and AOD care model within one of four local hubs.

The role involves direct client engagement, as well as overseeing and implementing targeted brief intervention processes to support clients throughout their care journey, ensuring seamless transitions and aftercare. The Stepped Care Lead will work closely with internal and external stakeholders, maintaining key relationships and service pathways to enhance community-based responses.

Responsibilities include:

  • Offering assistance to clients upon their first interaction with hub services, ensuring it is a welcoming and supportive entry experience.
  • Overseeing collaborative care processes, including progressive care planning, care transitions, and care-team reviews.
  • Offering overlapping support to clients moving to or from programs, implementing brief intervention aftercare models, and fostering sustained recovery.
  • Building and maintaining key relationships, service pathways, and agreements with partners of the local hub as well as work with the local lived experience workforce to build community-based responses.
  • Monitoring the results and demand related to the care model, coordinating effective collaborative responses to local needs in partnership with other stakeholders.
  • Reporting and Record Keeping.

About you 

We are looking for someone with strong organisational skills to effectively lead and represent the hub’s model of care.

The ideal candidate will have experience in multiple mental health and/or AOD (Alcohol and Other Drugs) settings, including knowledge of related systems, processes, legislation, and experience implementing brief-intervention approaches. Knowledge of the mental health sector and community needs is essential, along with a commitment to delivering person-centered care that respects and values individuals’ identities, backgrounds, and life experiences. A strong understanding and commitment to cultural safety and trauma-informed care principles are also required. Experience working independently and with care-team and community-based recovery approaches is preferred.

An ability to work collaboratively and professionally with other services and professionals across a range of life areas and needs, as well as a knowledge of the mental health sector and services offered the community to support those with complex needs will also be essential to succeed in this role.

Qualifications

Qualifications in psychology, social work, mental health nursing, or a related field.
