Micah Projects provides a range of support and advocacy services to individuals and families.
Inclusive Health Partnership Cluster is responsible for enabling vulnerable people to have access to health and social services to maximise their ability to live independently in the community. The cluster teams cover working with people who are experiencing homelessness and who are vulnerable due to their health in social housing, boarding houses, or hotels.
Homefront is a collaboration between Micah Projects, Queensland Health’s Mater Hospital, and primary care providers.
The Homefront Program delivers outreach support that connects people with clinical care, community support, and housing by providing integrated care.
How you will make a Difference:
As a Support and Advocacy Worker you will work as part of the Homefront Team to provide support and advocacy services including intake, triaging, and assessing referrals, liaising with internal and external stakeholders (including hospitals), and providing administrative support where appropriate.
This is a full-time employment opportunity, with conditions and salary at Level 4 of the SCHADS Award.
To be considered for this role, you will have a relevant tertiary qualification and/or demonstrated experience in providing support and case coordination to people with complex needs.
Demonstrated engagement and interpersonal skills, ability and commitment to work collaboratively within a team and a positive attitude is also essential for this position.
