Your Opportunity
Midtown Macquarie Park is an integrated model of Housing and Support and provides tenants holistic support through trauma informed care.
The support coordinator will provide assistance to tenants and/or participants in the Compass Program with a focus on achieving independent living. This role will provide individualised support and also ensure they are connected within the community.
Your key responsibilities will be to:
- Respond to referrals to the service, and conduct thorough assessments to determine their eligibility and support needs and establish a case plan with the client and the other relevant stakeholders
- Provide evidence-informed case management sessions with clients inline with Mission Australia’s National Case Management Approach
- Contribute to the development and facilitation of group sessions to meet the needs of the service, clients and the local community
- Develop, implement and review individualised case plans for each client, addressing such issues as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships
- Establish record, monitor and review each person’s progress, in consultation with Program Manager
- Develop quality relationships with a range of key stakeholders including community organisations, rehabilitation providers and other health professionals to allow for the effective provision of support and information to clients
Requirements for Success
- Relevant degree or diploma with at least 1 years’ of case management experience
- Understanding of social and/or affordable housing.
- Demonstrated ability to provide specialised or complex therapeutic interventions and manage a caseload of complex Consumer.
- Experience with clients dealing with issues including homelessness, domestic violence, poverty, abuse, mental health and the use and misuse of alcohol and other drugs, and knowledge of support services
- Highly developed administrative skills along with well-developed written and oral communication
- An understanding from combined lived experience and professional development about factors that support recovery and a proven ability to use this to positively support others on their recovery journey
