The Support Coordinator must apply highly attentive interpersonal skills, an in-depth understanding of the NDIS, the mainstream, Community, Mental Health and disability sectors, and expertise in time management, planning and administration, to enable each NDIS participant to access the supports and services they need to lead everyday lives, and to reach their personal goals.

KEY RESPONSIBILITIES:

  • Managing a caseload of NDIS participants and delivering tailored and individualised supports based on the persons NDIS plan.
  • Identifying and communicating any risks and ensuring that all work is conducted within compliance of relevant laws and standards.
  • Adhering to all polices and procedures of Fighting Chance and maintaining a culture of teamwork.
  • Conducting initial assessments of participants, interpreting NDIS plans and health information.
  • Research and coordinating a range of support to suit each individual participants needs across multiple providers.
  • Strengthen the capacity of participants and to support understand NDIS services to them to make informed choices.
  • Building and maintaining high quality, professional relationships and working closely with family & support networks.

SKILLS AND EXPERIENCE

To be successful in this role, you will ideally have:

  • A relevant qualification and/or 2+ years previous experience in similar case management/support coordination, or other relevant areas in the disability or similar sector.
  • Demonstrated understanding of the National Disability Insurance Scheme (NDIS) and the needs of people with a disability.
  • A passion for (and/or experience with) working with people with disability.
  • High bandwidth for multi-tasking.
  • Excellent communication and teamwork skills with a high level of empathy and interpersonal skills.

REQUIRED TO

  • Possess the right to work in Australia
  • Possess or willing to obtain the new NDIS worker screening check and Working with Children Check
  • Possess or willing to obtain Provide First Aid & CPR
  • Possess a current unrestricted NSW driver’s licence
  • Be available and willing to work on weekends and evenings on occasion.
