We are looking for an experienced Support Coordinator to join us for 12 months to assist clients and families to connect with funded disability supports. In this rewarding role, you will be tasked with developing participants’ networks and coordinating supports from a range of sources. You will build capacity for Achieve clients and report back to the NDIA on outcomes and goals.

Today, we are a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.

About the role
Reporting to the Program Lead, this role oversees client plans to create an empowering and enabling environment for people with disabilities and maximise opportunities for the attainment of personal goals and lifestyles. This role will manage a participant portfolio and will lead case management, planning and coordination of services.
Some of the key responsibilities include:
- Liaising with participants and their families to define program funding allocations.
- Developing individual client budgets.
- Liaising with plan managers to establish appropriate claim processes and monitoring outcomes.
- Working with clients and their families to enhance supports and understand their service agreements and their responsibilities.
About you
- Demonstrated understanding of NDIS, principles of Active support and human rights-based approach to service delivery.
- Strong interpersonal and verbal and written communication skills.
- Experience in achieving client outcomes in multiple settings.
- Proficient Computer literacy – able to easily learn and use MS Office Suite and online systems.
- Practical experience in achieving community inclusion and ensuring services maintain appropriate standards.
- Relevant degree qualification or equivalent experience in support coordination as a service.
