About the Role
The Support Coordinator plays an integral role in the development and achievement of a participant’s goals to maximise opportunity and independence.
The role involves;
- Implement, monitor and manage the participant’s NDIS plan
- Source, assess and connect with both NDIS funded and mainstream services
- Liaise with stakeholders to coordinate services
- Respond and manage any crisis when required
- Monitor funds spent
- Provide implementation and progress reports to the NDIA
- Ensuring high quality record keeping and reporting is maintained
About You
To be successful in this role you’ll share Yooralla’s values and have:
- Very good knowledge of the disability sector/ NDIS key principles
- Highly developed communication and interpersonal skills, both written and verbal
- Strong report writing skills and attention to detail
- Relevant experience working within the Human Services, Disability or Allied Health area
- Effective time management and organisational skills
- Ability to build strong working relationships and establish ongoing rapport, both externally & internally
- Experience within a similar position is desirable but not essential
- Independence and proactive learning style and sound problem-solving skills
- Proficient computer skills in using word, excel and online platforms for case noting
- Qualifications in, or, candidates studying Psychology/ Social work or other disability or human services studies will be highly regarded
