Better Health Network (BHN) is looking for a dedicated and compassionate Support Coordinator to join our team. This role focuses on providing person-centered support coordination to individuals with a range of disabilities, with an emphasis on promoting inclusivity and diversity. You will be part of a supportive team dedicated to making a positive impact on the lives of our clients.

Duties and Responsibilities:

  • Develop and implement NDIS plans with individuals.
  • Provide support to individuals to achieve their goals.
  • Promote and build consumer independence through capacity building.
  • Have a clear understanding of the NDIS sector and disability.
  • Work with individuals to identify their needs and goals.

Requirements and attributes:

  • Previous experience with NDIS desirable.
  • Previous experience or qualifications in Mental Health or related fields.
  • Experience in providing recovery-focused and client-centered models of care.
  • Demonstrated ability to promote and build consumer independence through capacity building.
  • Excellent knowledge of holistic, client-centered models of care and values underpinning the NDIS, specifically Support Coordination.
  • A clear understanding of the NDIS sector and disability.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability to change.
  • Ability to maintain confidentiality.
