Better Health Network (BHN) is looking for a dedicated and compassionate Support Coordinator to join our team. This role focuses on providing person-centered support coordination to individuals with a range of disabilities, with an emphasis on promoting inclusivity and diversity. You will be part of a supportive team dedicated to making a positive impact on the lives of our clients.
Duties and Responsibilities:
- Develop and implement NDIS plans with individuals.
- Provide support to individuals to achieve their goals.
- Promote and build consumer independence through capacity building.
- Have a clear understanding of the NDIS sector and disability.
- Work with individuals to identify their needs and goals.
Requirements and attributes:
- Previous experience with NDIS desirable.
- Previous experience or qualifications in Mental Health or related fields.
- Experience in providing recovery-focused and client-centered models of care.
- Demonstrated ability to promote and build consumer independence through capacity building.
- Excellent knowledge of holistic, client-centered models of care and values underpinning the NDIS, specifically Support Coordination.
- A clear understanding of the NDIS sector and disability.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility and adaptability to change.
- Ability to maintain confidentiality.
