On Offer:
- Based in Caboolture, QLD
- Permanent, part-time (0.6 FTE) position available

Role Overview:
Support Coordinator:
To support people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation through:
- Strengthening their ability to coordinate and implement supports in their National Disability Insurance
- Scheme (NDIS) individualised funding packages
- Supporting their engagement with mainstream, formal and informal services
- Developing capacity and resilience in their network.
Recovery Coach:
- Developing a recovery-enabling relationship with people including their family and carers
- Supporting a person to develop and implement their individual recovery plan
- Coaching a person to increase recovery skills and personal capacity, including motivation, strengths, resilience, and decision-making
- Collaborating with broader systems including available services and resources
- Supporting people to become active participants in the NDIS
Requirements:
- Minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
- Experience in a human services environment, particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health.
- Experience in assisting participants to test their eligibility of the National Disability Insurance Scheme (NDIS) or transition them to other appropriate supports.
- Highly-developed communication, interpersonal and negotiation skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals.
- Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
- Excellent organisational and time management skills.
- Must be eligible to apply/currently hold a WWCC
- Must hold/be willing to apply for NDIS worker screening clearance
- Current Drivers License
