The Opportunity

As a Transition Worker, you’ll be at the heart of a high-impact team. Supported by an experienced team and local manager you’ll use holistic case management to provide specialist transitional support for First Nations people with multiple and compounding needs who are leaving NSW Correctional Centres in the Far West of NSW.

Using a throughcare model focusing on the complex transition from incarceration into housing, your role includes:

  • Project and client support: You’ll actively build positive relationships with Government, community and other services to source relevant support and housing solutions. Receiving referrals, holistic case management, project administration and troubleshooting will make up most of your day. You’ll approach every task with empathy, keeping our clients’ goals front and centre.
  • Creating community connections for clients: Using an outreach model pre- and post-release, you’ll play a vital role providing clients with practical and psychosocial supports. The goal? To ensure long-term housing solutions and reduce recidivism.
  • Data, administrative and assessment duties: While the majority of this job involves client-facing duties, data collection will be an important part of your role. Supported by the Team Leader and Senior Worker, you’ll conduct general administrative duties, Workplace Health and Safety monitoring, and updating and maintaining accurate case files and client management system.
  • Cultural safety: You’ll be part of a team of First Nations staff working in culturally appropriate ways in recognition of the unique cultural needs and over-representation of First Nations people in the criminal justice system.

Your usual work hours will be 8.30am to 4.30pm Monday to Friday, with occasional out-of-hours work, as required. The role provides outreach support to clients residing in Broken Hill, with regular travel to Wilcannia.

To join us, you’ll need:

  • To be of an Australian First Nations background with connections to local services and communities
  • Experience and skills in the provision of culturally safe, trauma-informed care for First Nations People
  • Knowledge and experience of working with complex and compounding needs such as homelessness, criminal justice system involvement, alcohol and other drug use, mental health and experiences of trauma
  • A current NSW Driver Licence
  • A current NSW Working with Children Check and Criminal Record Check (please note, a criminal record is not a barrier to being considered for a role with CRC)
  • Demonstrated cultural competency and knowledge of issues relating to local Aboriginal communities
