Duties and Responsibilities
- Working closely with individuals and families, clients are engaged with to assess their eligibility for mental health, practical and financial support, ensuring a comprehensive understanding of their needs.
- Understand the evolving needs of community clients, creating personalised plans that foster their independence, empowerment, autonomy, and dignity.
- Assist clients to make informed decisions regarding options and supports available to optimise their mental health, wellbeing and sense of connectedness to the community.
- Providing referral pathways and advocacy for clients to all relevant services where necessary.
- Strong collaboration with all stakeholders involved to ensure the safety and wellbeing of vulnerable and at risk clients in the community.
- Provide psycho education and wellbeing resources to clients including access to and monitoring of client progress within the This Way Up program.
Desired Skills and Experience
About you
- Tertiary qualifications in Social Work, Psychology, Social Sciences, or similar;
- Prior experience in case work, actively engaging with individuals and their families, utilising a person-centred, strengths-based, and trauma-informed approach;
- Ability to work therapeutically with a variety of client populations with complex mental health and psycho-social concerns;
- Exhibit an understanding of care plan development to effectively achieve client outcomes and goals.
- Experience in assessing and managing client risk issues, including child protection concerns, suicidal ideation, self-harm, domestic & family violence.
- Utilise a range of best practice preventative and therapeutic strategies to ensure client safety and well-being.
- Ability to work autonomously, taking initiative and making sound decisions. Additionally, demonstrate effective teamwork skills, collaborating with colleagues and professionals from different disciplines.
- Provide information to clients and their families regarding government and community resources, as well as eligibility for practical and financial support.
- Coordinate necessary services and monitor their ongoing effectiveness.
