About the Role:

As the Young Adults Outreach Case Manager, you will play a vital role in VincentCare’s Young Adults Outreach Support Services program. This program was established to provide support to homeless young people by assisting them to secure and maintain affordable housing and addressing unmet needs through an assertive outreach case management process.

Using a client-centered approach, you will work alongside young people to develop their comprehensive case plans and assist them in achieving their goals. The position supports all young people who access the Northern Community Hub whilst also providing specialist support to Aboriginal and Torres Strait Islander young adults in the form of a Portfolio.

Utilising previous experience, the Portfolio Case Manager will provide collaborative case management for young people who identify as Aboriginal and Torres Strait Islander who are experiencing homelessness. Additionally, you will provide support in addressing needs such as: trauma, alcohol and other drugs, physical illness, psychosocial disability and/or mental health.

About you:

You are an appropriately qualified professional with experience in providing comprehensive case management, planning and support to assist clients within a homelessness setting.

You have experience in maintaining constructive, empathetic, supportive and respectful working relationships particularly for an integrated service response. Your experience and ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people is second to none and has enabled you to achieve desired outcomes as defined by your clients.

Key Selection Criteria:

  • Experience in and a broad understanding of the homelessness services and community services sector
  • Experience in working with Aboriginal and Torres Strait Islander young people with multiple and complex needs
  • Demonstrated understanding of, and an ability to implement, case management principles including person-centred, strengths-based practice and trauma informed approaches
  • Demonstrated knowledge working with external agencies and the ability to create positive lasting relationships
  • Demonstrated ability to build and maintain positive relationships with young people and empower them to maximise their potential through the development of independent living skills and pro-social community engagement
  • Demonstrated ability to establish & maintain positive and productive working arrangements with a range of government and non-government stakeholders
  • Can demonstrate excellent interpersonal verbal & written communication skills and high computer literacy
