The Care Manager I (CMI) is responsible for the assessment of resident psychosocial needs and coordination of support services for low-income older adults. The CMI also plans for and monitors services and interventions ensuring provision of quality care.

B.A or B.S in Social Work or related field required

COMPENSATION:
Range: $72,000 to $74,000/annual
