Role overview:

  • Provide psychosocial support for children and families requiring more intensive support. This may include supports to increase a child and/or family’s capacity for social and community connection, independent daily living, financial management, physical health and wellbeing etc.
  • Develop and review Individualised Support Plans in collaboration with the child, family and other professionals as needed, to identify and plan the required psychosocial supports that are need for the child and family.
  • Provide advocacy or support self-advocacy for carers where appropriate.
  • Provide assistance, information and referrals for alternative services as appropriate.
  • Liaise with and develop collaborative working relationships with key service providers in order to ensure ease of access to other services for children and families as required.
  • Provide support in diverse settings, including outreach and home visitations as required.
  • Provide support in diverse modalities, including individual, family-based, and group-based formats as appropriate.
  • Relate to children and families in a manner which is relevant and appropriate to their developmental and cognitive level of functioning and provide services in a professional and friendly manner.
  • Maintain appropriate documentation regarding intervention and support provided.
  • Administer and record appropriate outcome measures as required.

Requirements:

  • Minimum of a Diploma in mental health or community services related discipline (ideally a completed Degree with full AHPRA registration in Psychology OR AASW registration as an Accredited Mental Health Social Worker.
  • Previous experience in a Support Worker, or related role.
  • Knowledge, experience and skills working with children and families who are at risk of or currently experiencing mental health concerns, social and community isolation, financial and housing difficulties, physical wellbeing concerns etc.
  • Demonstrated capacity to relate to clients, families and other stakeholders with dignity and respect.
  • Ability to develop partnerships and relationships with other services to establish referral pathways into and out of the service to reach vulnerable children, families and carers who may not otherwise engage with mental health or children’s services.
  • Demonstrated ability to develop, implement and review plans surrounding a child and family’s support and intervention goals, vulnerabilities, and current risk, including strategies to stabilise and increase capacity for ongoing independent management.
  • Previous experience working in a mental health service setting.
  • Ability to work with marginalised children and families, including but not limited to Indigenous Australians, people from CALD backgrounds, children and young people in families currently or have previously experienced homelessness, unemployment, domestic violence or other potentially traumatic events.
  • Ability to work within a team.
  • Ability to work autonomously and manage a caseload.
  • Highly effective communication and interpersonal skills.
  • Record keeping, case note and computer data entry skills.
  • Must be eligible to apply/currently hold a Blue Card
  • Must hold/be willing to apply for NDIS worker screening clearance
  • Current driver’s license
